Registering to Attend Jamestown Public Schools
Welcome to Jamestown Public Schools!
To begin the registration process for a student to attend any district school, please call our Central Registration Office between the hours of 8:30 AM and 3:00 PM at 716-483-4344.
To expedite your registration experience, please print and complete our student registration forms prior to your registration appointment. Links for printing the registration forms are listed under "JPS Required Registration Forms." Please call if you would like to pick up forms.
If you have questions about registration or are ready to schedule an appointment, please call our office at 716-483-4344. The Central Registration Office is located in our central administration building at 197 Martin Road.

Required Documents for Student Registration
Please bring all completed registration forms with you along with the following documents:
- Proof of Residence (one of the following):
- residential lease agreement
- home mortgage statement or tax bill
- signed statement of residence from property owner or primary tenant (also available in Spanish). This is only for families who are “doubling up,” living with another family. Proof of residence must be provided by the property owner or primary tenant
Other documents may be considered if the above are not available. Please see the registration packet for examples, or discuss the options with the registrar.
If unable to provide proof of residence, please complete the Supplemental Residency Questionnaire provided by the registrar.
- Proof of Student's Age (one of the following)
- certified birth certificate
- certified baptismal certificate

- Health and Medical Information, including:
- immunization records, issued by physician or clinic
- record of physical examination completed within the last year, if available
- The Dental Health Certificate form should be completed by the parent or guardian and the dental office at the time of the child's checkup. The form may be faxed to Pupil Services (716-483-4278) or brought to the school after the checkup.
- The following information may also be requested, if applicable:
- affidavit of parental relationship
- report card or transcript from previous school
- any other health, psychological, or other pertinent evaluations

JPS Required Registration Forms
If your child has an Individualized Education Plan (IEP) or 504 Plan
After registering your child in Jamestown Public School District, records will be requested from the previous school. Once records have been received, the Committee on Special Education (CSE) Chairperson will determine your child's placement based on review of the records.
If you have copies of the following documents, please bring them with you to Central Registration to aid in CSE placement:
- IEP
- psychologist report
- all evaluations
- health plan/medical concerns
